Please attach the following documentation:

Documentation of Death. Examples include a death certificate or obituary.
- The document must state the full name of the deceased and date of death.
Proof of Authority. Examples include Power of Attorney documentation or Executor of the Estate.
- The document must state the full name of the deceased, your full name (the requestor) and your relationship to the deceased.
A photo of your valid government issued ID. Accepted forms of ID:
- Driver’s license, national identity card, passport, military ID.
- The ID must be valid (not expired).
- Must be a photo – the ID cannot be scanned or be a digital ID.
- The ID cannot be digitally edited or altered. If there is sensitive information you would like to obscure, please use a physical item such as tape or a piece of paper.

Note: This is only used for verification. Once completed the documents are immediately and permanently redacted for your privacy.
